I-918 Supplement A, U Visa Petition for Qualifying Family Member [pdf]

In December of 2021 United States Citizenship and Immigration Services released a new U Visa Qualifying Family Member Form to be used by applicants. Form I-918 Supplement A: U Visa Petition for Qualifying Family Member.
